The following will appear:

ENTER SECURITY (FUND):

2. Enter the SACUS, TICK or CUSIP for the security

ENTER DIVIDEND RATE ($ PER SHARE):

3. Enter the dividend amount that is being paid

The following will appear:

ENTER RE-INVEST PRICE:

4. Enter the price at which the dividend should be re-invested

The following will appear:

AS OF DATE (MM/DD/YY) - HIT ENTER FOR CURPRDATE :

5. Enter the date on which holders should own the position to be entitled to this dividend
